Members of Medway Dementia Action Alliance (DAA) have joined forces to help people diagnosed with dementia access a specialist support worker.
A Dementia Support Worker is someone who has been especially trained in Dementia; so that they can provide well-informed support and advice directly to people who are living with Dementia; they can also provide support for those who care for people with Dementia and their loved ones too.
This support has been produced by Medway Council and Medway Clinical Commissioning Group (Medway CCG), with a range of partners and agencies that includes Alzheimer's Society, Carers First, Imago, Medway Community Healthcare (MCH), Kent and Medway Partnership Trust (KMPT) and Age UK. They have teamed up to provide support to people with a diagnosis of Dementia and their loved ones; and to make sure that these specialist support workers are available.
Coordinating and working closely with Medway DAA and its range of partners, a Dementia Support Worker is someone who will work to assist people diagnosed with Dementia maintain their independence, and allow them the ability to make choices and stay in control of their choices for as long as needed.
