Vacancy - Community Life Planning Coordinator at wHoo Cares

Applications are now being taken for a Community Life Planning Coordinator now available at Hoo Peninsula Cares.

Hoo Peninsula Cares CIC (wHoo Cares) is a community based organisation with the vision to address social isolation and improve the health and well-being of vulnerable people of all ages.

They work to empower the local community to manage and support themselves, as well as help them to make the most of their local abilities, skills and expertise through our volunteer programme.

Supporting individuals on the Hoo Peninsula (ME3 postcode), wHoo Cares works with local communities, groups and organisations. They also work with other local and national organisations, including Medway Council.

They are looking for a dynamic and qualified health care or social care professional to be their Community Life Planning Coordinator (under unique circumstance, they will consider someone who can demonstrate the relevant experience without recognised qualifications).

Based in their offices on Isle of Grain, you will be part of a small and innovative team, working in partnership with key stakeholders to support health and social care initiatives.

Working closely with their Volunteer Coordinator, you would match volunteers with Partners to enable support in the community. In addition, you would maintain and develop working relationships with existing local authorities for referrals and provide desired responses to given situations.

The deadline for applications is 19th April 2017.

This is a prime opportunity for someone who has experience in helping people to identify, shape and access meaningful activities to improve feelings of well-being and make a difference in people’s lives.
