Continuing to Improve Health Outcomes – The Community Health Catalyst Programme from MVA

The Community Health Catalyst Programme, delivered in partnership between MVA and Swale CVS, continues to work towards its goal of improving the health prospects for local people in Medway and Swale - and they have recently shared some of their interim headlines.

The Programme works to engage with and listen to people that are experiencing health inequalities, particularly those within the CORE20PLUS5 communities in Medway and Swale.

Funded by Medway & Swale Health and Care Partnership (HCP), the aim is to find out how local health and care providers and similar groups can better work together to improve health outcomes.

Since its launch in Autumn of 2022, The Programme has been able to work directly with 20 VCS organisations in the immediate area, and has signposted to over 40 voluntary and statutory providers for health and wellbeing services.

And these services were able to deliver solutions which were much needed at that time like mental health support, health screening, smoking cessation, cost of living advice, befriending services, and social activities.

This work is set to carry on for another year, continuing to engage and improve the health prospects for local people in Medway and Swale.
